Updated Claims Instructions

You can find instructions for submitting claims in Section 12 of the Provider Manual. These instructions are intended to give you a step-by-step guide for entering the correct information, as required, to avoid any delay in processing.

Please note hand-written claims are scanned into an electronic format for processing. Therefore, it is imperative that you type or print clearly all information within the boxes provided, in the format indicated in these instructions. Adding additional information into the boxes will also cause a delay in processing. For example, a maximum of 6 sessions can be billed on each CMS (HCFA)-1500 form. If you are submitting more than 6 sessions, you will need to submit the additional sessions on a separate CMS (HCFA)-1500 form. That form must also be competed in its entirety-completing the sections with "SAME" or "SEE PREVIOUS" is not acceptable.
